Your personal information is collected on this form by Oakville Hydro Electricity Distribution Inc. (Oakville Hydro) under the authority of the Electricity Act, Section 29, the Ontario Energy Board Act, Section 28.3 and the Municipal Act, 2001, S.O. 2001, Section 11; and Halton Region By-law No. 71-19.
Your personal information will be used for the purposes of billing you for electricity services provided to you by Oakville Hydro, for billing you for water and wastewater use and collecting water and wastewater charges on behalf of Halton Region, and to respond to any questions that you may have regarding your bill.
Your personal information will be disclosed to Halton Region for the purposes of collecting unpaid water and wastewater charges, including collection through the addition of the unpaid charges to the property’s tax roll. Where you are a tenant, Oakville Hydro and/or Halton Region may disclose your personal information, including your water or wastewater billing history or arrears, but not your banking information and forwarding address, to the property owner for the above stated purposes.
Halton Region is not precluded from pursuing its legal remedies against the Tenant(s).
Your personal information may be collected from or disclosed to a consumer reporting agency for the purpose of assessing your risk of non-payment or to aid in the collection of unpaid amounts owing by you to Oakville Hydro or Halton Region.
